CONTROLLED OVARIAN STIMULATION (COS) FOR IVF
The first major stage of IVF treatment is Controlled Ovarian Stimulation (COS).
Unlike natural conception where one egg develops monthly, IVF aims to stimulate the ovaries to produce multiple mature eggs to improve fertilization and embryo development.
More mature healthy eggs may increase the chances of successful embryo formation and pregnancy.

MENSTRUAL PERIOD STARTS
Cycle Day 1
- First day of full menstrual bleeding
- Inform the fertility clinic immediately
- Schedule baseline assessment
- Prepare for medication

BASELINE SCAN & HORMONAL ASSESSMENT
Cycle Days 2-5
Pelvic Ultrasound To Evaluate
- Baseline follicles
- Ovarian cysts
- Endometrial lining
- Ovarian reserve
blood tests
May Include:
- Estradiol (E2)
- LH
- Progesterone
- Other hormones where indicated

START IVF INJECTIONS
Cycle Day 2ā5
Fertility injections are started to stimulate multiple follicles
Examples may include:
- FSH injections
- HMG injections
Typical Duration 8ā14 days
This increases the chance of:
- More eggs collected
- Better embryo development
- Better pregnancy success

SERIAL FOLLICULAR MONITORING
Cycle Day 6-14
During stimulation you will attend several ultrasound scans
These scans monitor:
- Number of follicles
- Follicle growth
- Ovarian response
- Endometrial thickness
You may require scans every 2ā4 days
18ā22 mm

TRIGGER INJECTION
Usually Cycle Day 10-16
Purpose:
- Final egg maturation
- Precise timing of egg collection
Egg collection usually occurs
34 - 36 Hours Later
IMPORTANT: BEFORE EGG COLLECTION (OPU)

You may be required to: Fast for 6ā8 hours before the procedure.

Medication Instructions
Take only medications approved by your fertility specialist.

Avoid
ā Heavy meals ā Alcohol ā Smoking

OVUM PICK-UP (OPU) / EGG COLLECTION
Egg collection is a minor procedure performed under sedation. A specialized ultrasound-guided needle is used to collect eggs from ovarian follicles.
WHAT HAPPENS DURING OPU?

Sedation medication is given

Ultrasound guidance is used

Follicles are gently aspirated

Eggs are collected and sent to the embryology laboratory

Procedure Duration Usually: 15ā30 minutes
AFTER OPU
Mild symptoms are common:
- Mild cramping
- Mild bloating
- Mild spotting
Most patients go home the same day.
WARNING SIGNS AFTER OPU
Seek urgent review if you develop
- Severe abdominal pain
- Heavy bleeding
- Fever
- Severe bloating
- Difficulty breathing

These may suggest
Ovarian Hyperstimulation Syndrome (OHSS)
OHSS (OVARIAN HYPERSTIMULATION SYNDROME)
OHSS occurs when ovaries over-respond to medication
Mild Symptoms
- Mild bloating
- Mild discomfort
Moderate Symptoms
- Increased abdominal swelling
- Pelvic discomfort
- Nausea
Severe Symptoms (Urgent Review Needed)
- Breathing difficulty
- Severe swelling
- Severe pain
- Dizziness
